How you can plan ahead (Preplanning) - Application Form

To arrange a prepayment plan, select either of the following options:

  • Option 1
    Call CACS at 1-877-738-2736 to arrange to meet with a representative at our office or at your home to arrange for a cremation.
  • Option 2
    Fill out the following form and submit to CACS. A representative will contact you to review your preplanning information and for you to obtain a price for the cremation of your companion and details of the monthly payments.
